Health tools overview
The following resources can help you manage your health and wellness:
- Health assessment – The health assessment identifies your health
risks and transition of care needs through a simple online questionnaire. Completion of the online or
paper health assessment within 90 days of your enrollment qualifies you for a $50 Visa card, which will
be sent to your home address within 3 to 4 weeks. You can update your assessment every six months.
The $50 Visa card is a one-time-only reward for the completion of your initial health assessment.
